The Horizon Home Care & Hospice Team devotes their expertise to comfort and care for individuals and families who are facing death and/or to those who are homebound. The agency also focuses on the grieving person by offering bereavement resources, counseling opportunities and a safe haven for those in need of comfort.

FRIENDLY VISITOR VOLUNTEER
"I’ve got a part-time job, 15 credits and an average of 140 English pages to read over the weekend but, I’ll make time to volunteer. The relationship with my Horizon family is too valuable and it builds every week," states Christopher Seidling, Friendly Visitor Volunteer.
Friendly Visitor Volunteers truly enjoy the relationship created with their assigned hospice patients and families. The commitment as a Friendly Visitor Volunteer is to assist in a variety of ways, such as, offering warm companionship to patients, assisting with house hold projects and errands, relieving stress for the care giver, facilitating a Life Review Project, and helping to create an atmosphere that allows the patient to feel comfort & dignity during this incredibly challenging and unique period of life.
Volunteers commit 1-2 hours a week to visiting with hospice patients and families. Consistent schedules are set by both the assigned family and the volunteer. Horizon asks that Friendly Visitor Volunteers are flexible.
All Friendly Visitor Volunteers are treated like Horizon employees and are required to be (18) years of age, have proof of automobile insurance and a vehicle. The screening process for Friendly Visitors consists of an interview, orientation, making a medical visit, and criminal & driving record background checks.

Our Bereavement Volunteers enjoy welcoming guests who enter the Horizon Grief Resource Center, assisting in bereavement telephone calls and offering assistance to our Bereavement Counselors. It is due to the excellence of our Bereavement Volunteers that Horizon is able to extend our arms to the community even more, by aiding individuals with more group grief sessions, individual counseling, spiritual reflection & stress relief programs, and more!
We ask that volunteers who commit to working in the Grief Resource Center are able to communicate compassionately and maturely. We also request that the volunteer is reliable.
Bereavement Volunteers must recognize that there will sometimes be down time in the Grief Resource Center. If a project is not readily available for our Bereavement Volunteers, we ask that they are able to work on personal projects.

MEALS-ON-WHEELS VOLUNTEER
Our Meals-On-Wheels Volunteers are exceptional individuals or civic organizations who dedicate their time to delivering warm & nutritious meals to patients who are homebound or elderly. If you are someone who enjoys a routine, making a connection with people and can offer (1) day a week to help out the Meals-On-Wheels Program, we need your assistance!
Volunteer availability is required from 10:30am to 2:00pm, (1)day a week / (2) days each month. Horizon needs consistent and reliable volunteers to offer their services.

COMFORT SHAWL VOLUNTEER
Horizon is fortunate to have many talented volunteers partake in the Comfort Shawl Program, a program in which volunteers create and donate shawls for hospice patients. Participants weave their care, their thoughts and/or their prayers into the colorful yarn of these shawls. The final product is delivered to patients by our hospice nurses with an uplifting message.
This program is a way to give the gift of love from one stranger to another. Be assured that Horizon patients are always overjoyed to receive these beautiful shawls from a caring individual in the community and, their families are also grateful to have a special keepsake in honor of their loved one.
If do not know how to knit, please join volunteers every Thursday evening to participate in Horizon’s newest Knitting Circle. Volunteers meet in the Grief Resource Center where a knitting mentor is available for instruction. It is a night of relaxation, education and laughter.

EVENT VOLUNTEER
Join Horizon at the Pfister this upcoming February, 21, 2009 for our second annual Gala Fundraiser. We are looking for Event Volunteers to help with registration, auction assistance, greeting guests, set-up and take-down. This is an event you don't want to miss offering your services at!
Event Volunteers must fill out an application and receive a criminal background check.
FLOWER ARRANGEMENT VOLUNTEER
This is a volunteer opportunity that allows time for personal reflection and creativity! Flower Arrangement Volunteers pick up flowers from Trader Joe's, Bay Shore Mall, and return to Horizon to arrange beautiful bouquets for patients. Volunteers can make consistent weekday schedules that match their own availability.

CLERICAL VOLUNTEER
Spend time in Horizon's Volunteer Center to put together training binders or admission packets, send out mailings, file important documents, or assist in a variety of projects.
